The purpose of this form is to help Missouri Breaks determine if the proposed study is a good fit for the company and community. It will also be used to solicit feedback from Community Advisory Board members and local stakeholders. Responses should be written in plain language and focus on information most relevant to community members.

Who are the PI and partners that make up the study team? Include contact information for primary contact. (600 character limit)

What are the logistics of the study? Include sample size, target community, study duration, frequency and timing of research visits, measurements collected at research visits, and target community. Missouri Breaks has staffed offices on the Oglala Sioux Tribe, Cheyenne River Sioux Tribe, Spirit Lake Tribe, and in Rapid City, SD. (1,000 character limit)

What evidence is there that this study addresses a community interest, need, or stakeholder priority of the target community? (1,000 character limit)

How will participants benefit from this study? Are there other local stakeholders or programs that may benefit? (600 character limit)

What is your plan to build local capacity and make sustainable impact in the target community? (600 character limit)

What is the proposed funding mechanism, when is the proposal due, what is the award budget, and what % of the budget to you anticipate allocating to Missouri Breaks? (600 character limit)
